In order to cancel your current membership at North Dallas Crossfit, it is required that you provide at least a 7 day notice to us. By submitting this form, you understand that you will be charged/debited one additional month if you do not cancel within 7 days before the end of your billing cycle. You may use North Dallas Crossfit facilities until the end of your cancellation period. Please ensure that you have selected your desired cancellation date and entered your information correctly. Please note that simply submitting this form does not finalize your cancellation with North Dallas Crossfit - one of our staff members will get in touch with you as soon as possible to confirm cancellation. We're certainly sorry to see you go, and would love some feedback about how we can improve our experience.
